You may have options for where you have your outpatient procedure. You can use tools like&nbsp;<a rel=\"noreferrer noopener\" href=\"https:\/\/www.healthcarebluebook.com\/ui\/consumerfront\" target=\"_blank\">Healthcare Bluebook<\/a>&nbsp;and the&nbsp;<a rel=\"noreferrer noopener\" href=\"https:\/\/www.medicare.gov\/procedure-price-lookup\/\" target=\"_blank\">Medicare Procedure Price Lookup<\/a>&nbsp;(for Medicare patients) to find out your financial responsibilities to have a procedure performed at our facility.&nbsp; Many health insurance websites also provide information on where to find in-network services.<br><br><strong>For an estimate of what your insurance will cover and what will be your responsibility, please contact your insurance carrier directly.<\/strong> If you do not have insurance or need payment arrangements, feel free to call us <a href=\"tel:415-440-1100\">(415) 440-1100<\/a> for payment options.<\/p><\/div><\/div>\n\n\n\n<div style=\"height:50px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">Your Rights and Protections Against Surprise Medical Bills <\/h2>\n\n\n\n<div id=\"section-ga924a5\" class=\"wp-block-gutentor-e1 section-ga924a5 gutentor-element gutentor-element-advanced-text\"><div class=\"gutentor-text-wrap\"><p class=\"gutentor-text\">When you get emergency care or are treated by an out-of-network provider at an in-network hospital or ambulatory surgical center, you are protected from balance billing. In these cases, you shouldn\u2019t be charged more than your plan\u2019s copayments, coinsurance and\/or deductible.<br><br><strong>What is \u201cbalance billing\u201d (sometimes called \u201csurprise billing\u201d)?<\/strong><br>When you see a doctor or other health care provider, you may owe certain <strong>out-of-pocket costs<\/strong>, like a <strong>copayment<\/strong>, <strong>coinsurance<\/strong>, or <strong>deductible<\/strong>. You may have additional costs or have to pay the entire bill if you see a provider or visit a health care facility that isn\u2019t in your health plan\u2019s network.<br><br>\u201cOut-of-network\u201d means providers and facilities that haven\u2019t signed a contract with your health plan to provide services. Out-of-network providers may be allowed to bill you for the difference between what your plan pays and the full amount charged for a service. This is called \u201c<strong>balance billing<\/strong>.\u201d This amount is likely more than in-network costs for the same service and might not count toward your plan\u2019s deductible or annual out-of-pocket limit.<br><br>\u201cSurprise billing\u201d is an unexpected balance bill. This can happen when you can\u2019t control who is involved in your care\u2014like when you have an emergency or when you schedule a visit at an in-network facility but are unexpectedly treated by an out-of-network provider. Surprise medical bills could cost thousands of dollars depending on the procedure or service.<\/p><\/div><\/div>\n\n\n\n<h4 class=\"wp-block-heading\"><strong><strong>You\u2019re protected from balance billing for:<\/strong><\/strong><\/h4>\n\n\n\n<p><strong>Emergency services<\/strong><br>If you have an emergency medical condition and get emergency services from an out-of-network provider or facility, the most they can bill you is your plan\u2019s in-network cost-sharing amount (such as copayments, coinsurance, and deductibles). You can\u2019t be balance billed for these emergency services. This includes services you may get after you\u2019re in stable condition, unless you give written consent and give up your protections not to be balanced billed for these post-stabilization services.<\/p>\n\n\n\n<p><strong>Certain services at an in-network hospital or ambulatory surgical center<\/strong><br>When you get services from an in-network hospital or ambulatory surgical center, certain providers there may be out-of-network. In these cases, the most those providers can bill you is your plan\u2019s in-network cost-sharing amount. This applies to emergency medicine, anesthesia, pathology, radiology, laboratory, neonatology, assistant surgeon, hospitalist, or intensivist services. These providers can\u2019t balance bill you and may not ask you to give up your protections not to be balance billed.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><strong>You\u2019re NEVER required to give up your protections from balance billing. You also aren\u2019t required to get out-of-network care. You can choose a provider or facility in your plan\u2019s network.<\/strong><\/h4>\n\n\n\n<p>If you get other types of services at these in-network facilities, out-of-network providers can\u2019t balance bill you, unless you give written consent and give up your protections.<\/p>\n\n\n\n<p>California: Comprehensive Balance Billing Protections<\/p>\n\n\n\n<div id=\"section-g8801f1\" class=\"wp-block-gutentor-e7 section-g8801f1 gutentor-element gutentor-element-list\"><ul class=\"grid-row gutentor-grid-item-wrap list-indicator-icon\"><li class=\"gutentor-list-item grid-lg-12 grid-md-12 grid-12\"><span class=\"gutentor-single-item gutentor-single-item-0\"><span class=\"gutentor-list-item-wrap g-s-list-wrap\"><span class=\"gutentor-single-list-item-icon\"><i class=\"list-item-icon fas fa-arrow-alt-circle-right\"><\/i><\/span><span class=\"gutentor-list-item\"><p class=\"gutentor-single-item-title\">State requires insurers to hold enrollees harmless for amounts beyond in-network level of cost sharing<\/p><\/span><\/span><\/span><\/li><li class=\"gutentor-list-item grid-lg-12 grid-md-12 grid-12\"><span class=\"gutentor-single-item gutentor-single-item-1\"><span class=\"gutentor-list-item-wrap g-s-list-wrap\"><span class=\"gutentor-single-list-item-icon\"><i class=\"list-item-icon fas fa-arrow-alt-circle-right\"><\/i><\/span><span class=\"gutentor-list-item\"><p class=\"gutentor-single-item-title\">States prohibits out-of-network providers from billing enrollees for any amount beyond in-network level of cost sharing<\/p><\/span><\/span><\/span><\/li><li class=\"gutentor-list-item grid-lg-12 grid-md-12 grid-12\"><span class=\"gutentor-single-item gutentor-single-item-2\"><span class=\"gutentor-list-item-wrap g-s-list-wrap\"><span class=\"gutentor-single-list-item-icon\"><i class=\"list-item-icon fas fa-arrow-alt-circle-right\"><\/i><\/span><span class=\"gutentor-list-item\"><p class=\"gutentor-single-item-title\">Above protections apply:<br>\u25cf   To HMO and PPO enrollees<br>\u25cf   For (1) emergency services by out-of-network professionals and facilities and (2) non-emergency services provided by out-of-network professionals at in-network facilities<br>\u25cf   Provided by all or most classes of health care professionals<\/p><\/span><\/span><\/span><\/li><li class=\"gutentor-list-item grid-lg-12 grid-md-12 grid-12\"><span class=\"gutentor-single-item gutentor-single-item-3\"><span class=\"gutentor-list-item-wrap g-s-list-wrap\"><span class=\"gutentor-single-list-item-icon\"><i class=\"list-item-icon fas fa-arrow-alt-circle-right\"><\/i><\/span><span class=\"gutentor-list-item\"><p class=\"gutentor-single-item-title\">State provides a payment standard<\/p><\/span><\/span><\/span><\/li><li class=\"gutentor-list-item grid-lg-12 grid-md-12 grid-12\"><span class=\"gutentor-single-item gutentor-single-item-4\"><span class=\"gutentor-list-item-wrap g-s-list-wrap\"><span class=\"gutentor-single-list-item-icon\"><i class=\"list-item-icon fas fa-arrow-alt-circle-right\"><\/i><\/span><span class=\"gutentor-list-item\"><p class=\"gutentor-single-item-title\">Protections do not apply to:<br>\u25cf  ground ambulance services<br>\u25cf  enrollees who consent to non-emergency out-of-network services<br>\u25cf  enrollees in self-funded plans<\/p><\/span><\/span><\/span><\/li><\/ul><\/div>\n\n\n\n<p><\/p>\n\n\n\n<p>When balance billing isn\u2019t allowed, you also have these protections:<\/p>\n\n\n\n<div id=\"section-g990e57\" class=\"wp-block-gutentor-e7 section-g990e57 gutentor-element gutentor-element-list\"><ul class=\"grid-row gutentor-grid-item-wrap list-indicator-icon\"><li class=\"gutentor-list-item grid-lg-12 grid-md-12 grid-12\"><span class=\"gutentor-single-item gutentor-single-item-0\"><span class=\"gutentor-list-item-wrap g-s-list-wrap\"><span class=\"gutentor-single-list-item-icon\"><i class=\"list-item-icon fas fa-arrow-alt-circle-right\"><\/i><\/span><span class=\"gutentor-list-item\"><p class=\"gutentor-single-item-title\">You\u2019re only responsible for paying your share of the cost (like the copayments, coinsurance, and deductible that you would pay if the provider or facility was in-network). This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.<br><br>Make sure your healthcare provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item.
